Description:
TheraSim CS is the industries only fully interactive patient case simulation tool for chronic and infectious disease diagnosis and treatment. TheraSim CS moves the medical learning experience closer to what physicians actually do – treat patients.
TheraSim CS is a true simulation wherein clinicians can order from thousands of labs, diagnoses and drugs using an intuitive, patient chart-based browser interface. By not limiting the physicians decisions to a multiple-choice approach, the data captured in TheraSim CS is a more accurate reflection of clinical behavior and is more predictive of actual clinical performance. TheraSim CS and it’s dashboard reporting tool, TheraSim HMD – Health Metrics Dashboard, are able to provide quantitative clinical performance metrics on skills related to appropriate diagnosis and treatment. TheraSim CS incorporates multi-media such as video diagnostics, video-based patient-interviews and video –based feedback from experts. Every diagnostic or treatment decision that the clinician makes for the patient is analyzed by TheraSim Clinical Rules and instant feedback is provided. This E-mentoring approach not only measures physician behavior but increases clinician competency and confidence.
TheraSim CS is designed to measure physician behavior and reinforce best practices in medical care delivery and drug development. TheraSim CS leverages a smart medical knowledgebase of best practices, pharmacokinetics and patient metabolic information to bring actual patient case data to life as advanced medical simulations. TheraSim works with experts, investigators and faculty to update its knowledge base with the latest best practices, drug data and patient cases from reputable, peer-reviewed sources.
In a dynamic clinical environment, practitioners review real patients’ histories, order laboratory tests and procedures, interpret findings, render diagnoses, and use drug therapies. In the Clinical Trial version of TheraSim CS, CRA’s, investigators and site personnel can screen patients for enrollment, administer the investigational product, manage and report on adverse events and other protocol related tasks. Every action taken and every preference indicated in the course of rendering diagnoses, drug selection and dosing is recorded and analyzed. Feedback to the clinician includes alerts for all errors as well as cited resources for further exploration and learning. (see figure A)
This approach offers a unique value to the practicing community as TheraSim CS is the only case based approach that does not limit physician choices for diagnosis and treatment. This approach creates a truly objective and realistic learning event for physicians and other care providers. Recent pilots have proven out the effectiveness of TheraSim Patient Simulator’s approach in engaging clinicians. Activity completion rates typically range from 50% - 70% and user satisfaction exceeds 95%. |

| TheraSim Patient Simulator: Directed Learning Resource |
| TheraSim tracks and measures all user decisions against a database of cited clinical resources that include peer reviewed reports and trials, published expert panel guidelines, package inserts, and relevant abstract. If user decisions differ from the database reference materials and the difference is contrary to good medical practice, a scoring deduction will be marked citing the relevant materials. At the end of the session, these cited materials may be complied and made immediately available to the user. Reference materials may include the following: |
| |
Full article, where in the public domain or made freely available by the copyright holder (e.g., published guidelines, conference abstracts, clinical trial data, and Package Inserts) |
Abstract, where item is not in the public domain or freely available or (e.g., symposia abstracts, published articles) |
Citations for articles where no portion is in the public domain or freely available (e.g., certain copyrighted articles) |

On going needs assessment
Skills gaps as determined by the scoring mechanism above are applied to the evaluation of achievement of pre-determined learning objectives. From this data, skills gaps reports are created and used as a needs assessment in the development of future continuing education activities
For chronic diseases or those presenting with complications, co morbidities and opportunistic infections, TheraSim Patient Simulator will follow the patient encounter through time with several visits per year. The emphasis of the on going treatment programs will be continuum of care as well as emphasis on major areas of skills gaps from previous programs.
| |
| |
| In addition to tracking overall metrics for performance, the TheraSim Clinical Simulator provides a quarterly report separately summarizing the change in performance amongst those returning program users in order to best access levels of change in competencies for diagnosis and treatment. |
The TheraSim CS interface is designed around the patient chart. Presented in an electronic format, TheraSim CS is easy for clinicians to navigate and understand. Little to no instruction is required to navigate TheraSim CS interface.
This is the Patient Intro tab where the clinician user meets the patient, reads the primary complaint and reviews photos and video clips of physiological conditions that are evident in the file or the physical exam. |
On the TheraSim CS 'History Tab', the clinician can review the medical history of the patient, including physician's notes, vitals, etc. |
| |
| On the TheraSim CS 'Labs Tab', the clinician can select from dozens of labs for the patient including medical images such as x-rays, smears and even video-based labs such as echo-cardiograms. Standard labs such as chem-screens and CBCs are also available. |
The 'Lab History Tab' summarizes all the critical lab data. It shows longitudinal markers as graphs with the drug therapy underneath for easy cross reference. |
On the 'Diagnosis Tab', the clinician chooses from hundreds of potential diagnoses for the case. Unlike other case tools, TheraSim CS does not limit the clinician choices to 3 or 4. In an actual clinical encounter, a clinician has thousands of diagnosis and therapy choices for the patient, just as they do in TheraSim CS. As a result, the data that TheraSim CS collects is a more accurate reflection of clinical practice.
| Once a diagnosis is selected, TheraSim CR provides instant feedback about diagnosis, disease staging and other considerations. Messages from the TheraSim CR - Clinical Rules engine are displayed on the right side of the screen under "Clinical Guidance". |
On the 'Order Tab', the clinician builds a therapy for the patient. Choosing from a list of hundred of medications, the clinician has a full list of therapeutic options to choose from. When a medication is selected, TheraSim displays all available dosing forms for the user. Once medications are prescribed, the TheraSim CR - Clinical Rules engine analyzes the fitness of that therapy for the specific patient case under consideration using a knowledgebase of clinical treatment guidelines and pharmacology and displays custom warnings and alerts on the right side |
| of the screen under 'Clinical Guidance'. TheraSim CR also provides convenient links to guidelines, drug prescribing information and other useful resources. In addition to providing warnings on such things as Therapy combinations and drug interactions, TheraSim CR also provides 'Management' text. All messages from TheraSim CR note the clinical references they come from. |
At the end of the session, TheraSim CS provides a detailed analysis of the clinician's performance. Links are provided so the clinician can drill down into areas for further review. A quantitative score is provides in each of 14 clinical categories such as; Diagnosis, Therapy Initiation, Drug Interactions, Warnings and Precautions, etc. The system also provides closing case remarks which are written by a panel of experts and peer to peer statistics which show how the therapy choices of the user compare to those of the their peers. |
| |
| TheraSim CS provides a huge database of clinical resources to assist the clinical in diagnosing and treating the patient. These resources include drug prescribing information and guidelines. |
